Best Practices for Logging Verbal Consent (For Internal Use)

Each time we obtain verbal consent, we log the following details in our CRM or contact database:

  • Full Name

  • Phone Number

  • Date and Time of Verbal Consent

  • Method of Consent: “Verbal – Recorded by [Team Member Name]”

  • Script Confirmed: “Yes – Verbal Script v1.0”

  • Notes (optional): e.g., "Consent obtained during discovery call on Zoom."

Metamorphosis CCT LLC (the “Company”) respects the privacy concerns of the users of its website, www.kajabi.com and the services provided therein (the “Site”).
